This book has been created by a retired Registered Nurse who has extensive experience working in various medical fields. Hospitals, medical centres, community, corporate telephone and flight nursing, to chronic disease management, surgical wards, medical wards and the Emergency Department.

I found throughout my career of nursing that, majority of patients or their carers that I would see had chronic illnesses that would see multiple health care professionals. From allied health care professionals such as physiotherapists, social workers, to nurses, GP’s, and Medical Specialists. It was difficult for patients or their carers to keep track of their chronic illness and the ongoing changes. When patients see one Doctor after another, patients can easily get confused or forget their ongoing care and treatments for chronic disease management.

Having found that nearly every discharge letter from a hospital or a referral letter from a Doctor has at least ONE incorrect detail of information about their patient.

I have based the design of this book from knowing the EMR ( electronic medical records ) system. It has been simplified and broken down into sections for quick access and reference. It’s format is to help you remember, keep track and follow through with ease. Take it with you to every medical appointment, for this book will not only help you or your carer, but it will also help medical professionals including ambulances, especially during an emergency.
